End of Course Evaluations (aka the new SAIS)

Every primary instructor of qualifying courses will receive the CORE set of evaluation questions.

Core Form

Courses with the following Schedule Descriptions in Banner are excluded from End of Course Evaluations because generally they do not meet the criteria that End Of Course evaluations are designed to measure. 

  • Field Study/Supervision
  • Personalized Self Instruction
  • Off-Campus Study
  • Praticum
  • Research (includes Dissertation and Thesis hours)
  • Use of Facilities

PLEASE NOTE:  If you believe your excluded course should be evaluated (such as Field Study/Supervision or Practicum courses that utilize classroom instruction in addition to external experiences) please contact Elizabeth Pemberton at  We will add these courses to EOC Evaluations at the instructor’s or department’s request.

Courses identified as having an online component in Banner will receive the supplemental ONLINE set of evaluation questions.

Online Form

Secondary instructors of courses with a lab session will receive the supplemental LAB set of evaluation questions.
Lab Form

Secondary instructors of courses with a discussion/recitation session will receive the supplemental DISCUSSION/RECITATION set of evaluation questions.

If you teach a class that utilizes a teaching assistant, but that teaching assistant cannot be listed in Banner, you may request the supplemental TA set of evaluation questions by contacting Elizabeth Pemberton at

If you are currently in the tenure and promotion process and would like to use the first four questions from the old SAIS forms, please contact SAIS at  These four questions will appear as a supplement to the new core set of questions.
